ASSISTED LIVING SERVICES
Our Lady of Fatima
provides service plans containing various levels of personal assistance and
care, depending on your needs
Levels of care are determined based on the number of personal care minutes
needed to provide the care required by the resident. The following are examples
of services that represent the approximate number of minutes in each level of
care
Level A
Our Lady of Fatima will provide you with:
• living accommodations, food service, minimal observation and supervision;
• regular housekeeping and laundry service;
• planned activities program;
• arrangements for shopping, medical and dental appointments within
10 mile radius
Level A-1
The services listed in Level A plus:
• assistance with self-administration of medication
Level B
The time required for services listed in Level A and A-1 which may include
intermittent care services and assistance with:
• medication management
coordination with physician for orders and prescriptions, delivery of
medications at prescribed times
• dressing
• grooming
• bathing
• escorting to and from
meals
• other basic activities of daily living as needed.
Level C
The time required for services listed in Level , A-1, and B which may include
more assistance with:
• getting up in the morning and preparing for bed at night
• night assistance with toileting or ambulation to toilets;
• treatments (i.e., wound dressings)
• care of diabetics on insulin
• incontinence care
• oxygen monitoring
Level D
The time required for services listed in Level , A-1, B and C which may include
extensive assistance with the activities of daily living such as:
• assistance with transfer
• other activities performed by the caregiver requiring extensive time
• frequent and excessive clean up of the person or apartment
• resistant behavior resulting in excessive time of the caregivers
